Staff Application
Items required to apply for a position at Passionate Care Services
– ALL DOCUMENTS MUST BE ORIGINALS –
- Level 2 Background Screening
- Local Background check from Police Dept. or Sheriffs office
- CNA, LPN, RN license or HHA Certificate
- Current Florida Driver’s License
- Original Social Security Card
- Resident Alien Card, if applicable
- Current Health and Physical within the last 6 months stating “Free from Communicable Disease”
- Current CPR Card
- Proof of Auto Insurance & Auto Registration
- Professional Liability Insurance
- Resume showing 2 years experience working in healthcare or with individuals with disabilities.
- Physical Examination within the past 6 months.
- State of Florida Approved Certifications in Continuing Education Unit:
- Patients’ Rights and Dignity
- Alzheimer’s Disease
- Communicating with the Cognitively Impaired Client
- Preventing Medical Errors
- Infection Control
- HIPAA (Health Insurance Portability and Accountability Act)
- HIV/AIDS
- Domestic Violence
- OSHA (Occupational Safety and Health Administration)
- Documentation
- Legal Aspects and Ethics
